アルツハイマー病のバイオマーカーとは？
2023年アルツハイマー協会国際会議（AAIC）では、AD診断基準の草案が発表され、初めて血液ベースのバイオマーカー検査が導入されました。ADは、Aβ42、Aβ40、P-tau181、P-tau217、P-tau231、NFL、GFAPといった血漿バイオマーカーによって診断できます（上記の表を参照）。これらのバイオマーカーはまだ大規模な臨床試験を受けておらず、関連するグループによるさらなる分析が必要です。ADの診断には、臨床症状や画像診断など、総合的な方法に依存する必要があります。

アルツハイマー病(AD)は、神経構造の維持に重要な役割を果たすタウタンパク質の異常蓄積を特徴とする神経変性疾患です。新たな研究では、リン酸化タウ断片、すなわちリン酸化タウ181 (p-tau181)
、リン酸化タウ217 (p-tau217)、リン酸化タウ231 (p-tau231)、p-tau202/205、p-tau212/214、p-tau413、およびp-tau422が、アミロイドベータ(Aβ)蓄積後に脳内で早期に増加することが示されています。これらの増加は神経原線維変化が検出される前の数年、および症状が現れる数十年前に起こります。これらのバイオマーカーの定量化は、単独のAβレベルよりも高い感度を示し、ADと他の認知症の形態をより正確に区別することを可能にします。特に、リン酸化タウ断片はADの進行に伴い血液中のレベルが10倍まで増加する可能性がある一方で、Aβ42/40比は約15%減少するだけです。
モノクローナル抗体、例えばAT8、AT100、AT180、およびAT270は、特定のタウタンパク質の形式を標的とするように開発されました。例えば、AT8はセリン202およびトレオニン205部位でリン酸化されたタウを特異的に認識し、ADにおける異常なタウタンパク質の蓄積の識別を支援します。AT100はトレオニン212およびセリン214部位でリン酸化されたタウを標的とし、ADの初期段階の神経原線維病理を検出することを容易にします。AT180はトレオニン231部位でリン酸化されたタウに結合し、タウタンパク質のリン酸化と神経変性の関係を理解するのに貢献します。さらに、AT270はトレオニン181部位でリン酸化されたタウを標的とし、AD患者の脳脊髄液中のタウタンパク質レベルの上昇を検出することが示されています。これらのモノクローナル抗体は、ADのような神経変性疾患の病理メカニズムを調査するための貴重な研究ツールとして役立ちます。
